The torsional deformation behavior of an elastic bar with a circular cross-section was investigated by applying invariant dyadic analysis, where the small finite displacement functions advocated by Saint-Venant (1855) were fully employed. It was found that the previously overlooked circumferential shear force field generated by pure torsion on the side walls of a bar produces an unusual torque term induced by the skew-symmetric part of the deformation tensor and exhibits quadratic length dependence along the z-axis of the bar. The adaptation of this torque term for a helical conformation of α-peptides creates moments acting on the circular cross-sections and is directed along the surface normal of circular cross-sections, which coincides with the tangent vector of the helix. The projection of this torque along the z-axis of the helix varies quadratically with the azimuthal angle. The radial component of the unusual torque, which also lies along the principal normal vector of the helix, starts to perform a precession motion by tracking a spiral orbit around the z-axis, whereas its apex angle decreases asymptotically with the azimuthal angle and finally reaches a finite value depending on the height of the helix along the z-axis. The ordinary torque terms, which are also deduced from the self- and anti-self-conjugate parts of the deformation tensor, have magnitudes half that of the full torque term reported in the literature. The present results were applied to the helical conformation of α-peptides designated by {3.611} to show that the mechanical stability of strained open-ended helical conformations can be successfully achieved by spontaneous readjustments of the surface and bulk Helmholtz free energies under isothermal isochoric conditions. It has been demonstrated that the main contribution to the mechanical stability of α-peptide 3.611 cannot come alone from the electrostatic dipole-dipole interaction potential of the anti-align excess dipole pairs but also from the surface Helmholtz free energy, which is characterized by a binding free energy of -15.5 eV/molecule (-32.56 Kcal/mole) for an alpha-peptide composed of 11 amino acid residues with a critical arc length of approximately 10 nm, assuming that the shear modulus is G = 1GPa and the surface Helmholtz specific free energy density is fs = 800 erg/cm2. This result was in excellent agreement with the experimental observations of the AH-1 conformation of (Glu)n Cys at pH 8. The present theory indicates that only two excess permanent anti-align dipole pairs for one α-Helical peptide molecule is requirement to stabilize the whole secondary structure of the protein that is exposed to heavy torsional deformation during the folding processes which amounts to 7.75 eV/molecule stored electrostatic energy compared to the interfacial Helmholtz free energy of -23.25 eV/molecule, which is exposed to hydrophobic environments.
A prospective cross-sectional study of diabetic patients registered at the Eye department of Tobruk Medical Center in Tobruk-Libya between (01.06.2023 to 29.02.2024). At the same time data was collected in the eye department in Alwahda Derna Teaching Hospital, Data was collected using the same administered questionnaire. Grading of DR was done by slit-lamp examination and Fundoscopy by an ophthalmologist. Descriptive analysis included percentage mean, median, and p - value by using SPSS 25. There were 185 diabetic patients, ages ranging between 30 and 90 years, with a mean of 58, the majority (97.6%) were of Libyan nationality. There were 86 males and 99 females. Patients with a duration of diabetes ranging between 11 and 15 years were at almost a five-fold risk of developing DR compared to those with a duration of < 10 years. Most of advanced complicated diabetic retinopathy cases had diabetes for more than fifteen years, and no advanced disease in the cases with a duration of less than 10 years.

Pneumomediastinum (mediastinal emphysema) is an uncommon condition characterized by the accumulation of air or gas in the mediastinum. Here is a case of a 16-year-old female known to have depression who presented to the emergency department with complaints of shortness of breath, restlessness, chest discomfort, and hoarseness of voice for 2 days. She was initially diagnosed with panic attack, and later on clinical examination, surgical emphysema over the right supraclavicular area was noticed. Chest X-ray was found to be normal, and further imaging with high-resolution computed tomography (HRCT) of the thorax showed pneumomediastinum. In this report, the clinical presentations, radiological features, and management of pneumomediastinum will be discussed.
The causes of hypercapnia in Acute Cardiogenic Pulmonary Edema are still unknown. Our hypothesis recognizes an alteration of the ventilation-perfusion ratio in the apical areas of the lung as the major cause. The redistribution of blood to the apical lung zone in hypertensive pulmonary circulation determines an excess of perfusion compared to ventilation and consequently hypercapnia. Our suggestion might be in favor of Bilevel-PAP over Continuous Positive Airway Pressure (CPAP) as the first line of therapy.
This study explores the application of a Multi-Criteria Decision-Making (MCDM) approach based on q-rung orthopair fuzzy numbers to identify the origin of mechanical parts found at vehicle accident scenes. The primary objective is to determine the most likely vehicle to which these parts belong by evaluating key criteria such as compatibility, damage level, serial number matching, and color compatibility. Q-rung orthopair fuzzy numbers offer an advanced method to handle the inherent uncertainty and vagueness associated with forensic evaluations, particularly in scenarios where data is incomplete or imprecise. The proposed methodology involves defining the criteria, assigning membership and non-membership degrees using q-rung orthopair fuzzy sets, and applying an aggregation process to effectively rank alternatives. This approach facilitates flexible decision-making by accommodating different levels of confidence and uncertainty, making it particularly suitable for forensic applications where evidence is often ambiguous. The findings demonstrate that integrating q-rung orthopair fuzzy numbers into the MCDM framework significantly enhances the accuracy and reliability of identifying vehicle components involved in accidents. The proposed methodology provides a systematic tool to support forensic investigations, aiding in the determination of liability and contributing to more robust outcomes in mechanical evidence analysis.
This paper presents the results of experiments to investigate the influence of thermal characteristics of the environment on the thermal modes of transparent boxes. To conduct experiments on the non-stationary thermal model of solar greenhouses developed by us, two identical transparent boxes with dimensions of 0.80 x 0.65 x 0.80 m were constructed. The transparent boxes have rectangular shapes. One transparent box has glass walls and the other with polyethylene walls. The influence of the thermal characteristics of the environment and the thermal conditions inside the transparent boxes with film and glass transparent walls are investigated. The experimental results show that at a maximum ambient air temperature of 42 °C on 27.06.2024 at 13:48 hours, the air temperature increases to 10% and 23% in transparent boxes with polyethylene and with glass walls, respectively, and at 05:10 hours, the humidity decreases to 8% and 11%, respectively. Thus, the influence of the thermal characteristics of the environment on the thermal conditions of transparent boxes with glass walls, at the maximum ambient temperature, is greater by 1.2 times than in transparent boxes with polyethylene walls, and humidity decreases by half.

Sleep disorders represent a significant public health concern due to their widespread prevalence, impact on overall health, and the economic burden they impose. These disorders encompass a broad spectrum of conditions, ranging from insomnia and obstructive sleep apnea (OSA) to narcolepsy, restless legs syndrome (RLS), and parasomnias. They are often associated with comorbidities such as cardiovascular diseases, metabolic dysfunctions, and mental health disorders, making their identification and management critical.The publication of this work is of high interest as it contributes to the expanding body of literature focused on understanding the complex interplay between sleep disorders and health outcomes. By presenting detailed case reports, this study provides valuable insights into the diagnostic challenges, treatment modalities, and potential avenues for personalized interventions in sleep medicine. Case reports are particularly important in this field as they shed light on unique presentations and rare conditions that might otherwise go unnoticed in large-scale epidemiological studies. From an epidemiological perspective, sleep disorders are highly prevalent globally. According to the World Health Organization (WHO), approximately 30% - 45% of the global population experiences sleep disturbances. This paper presents three representative case reports: chronic insomnia, severe OSA, and narcolepsy with cataplexy. Each case is analyzed in-depth, highlighting patient history, PSG findings, treatment interventions, and outcomes. For chronic insomnia, cognitive-behavioral therapy for insomnia (CBT-I) and pharmacological intervention resulted in marked improvements in sleep latency and efficiency. In the OSA case, continuous positive airway pressure (CPAP) therapy significantly reduced the apnea-hypopnea index (AHI) and alleviated daytime symptoms. The narcolepsy case demonstrates the efficacy of modafinil and sodium oxybate in managing excessive daytime sleepiness and cataplexy.Despite advancements, challenges persist in the field, including patient adherence to therapy, accessibility to specialized sleep studies, and the ethical implications of AI-driven diagnostic tools. Future research should focus on scalable, patient-centric approaches and the role of emerging technologies in enhancing diagnostic accuracy and treatment efficacy. This paper aims to contribute to the evolving understanding of sleep disorders, bridging clinical case insights with the broader implications for sleep health and research.

Visualizing a nodule in the liver parenchyma of a patient with chronic liver disease raises the suspicion of hepatic malignancy. We report here the case of a 63-year-old female with primary biliary cholangitis (PBC) in whom a hepatic pseudolymphoma (HPL) was incidentally detected. This fairly rare lesion mimics primary liver cancer, has no specific radiological features, and requires histology for a definite diagnosis. This tumor-like lymphoid liver proliferation has been reported in clinical situations with immune-mediated inflammation including PBC. It can be observed in many organs but very rarely in the liver. The diagnosis of HPL should be considered when detecting a liver nodule in a patient with this particular chronic cholestatic liver disease.

This study investigates the impact of socio-political disruptions on air quality in Khartoum, Sudan, focusing on key pollutants: Aerosol Optical Depth (AOD), Carbon Monoxide (CO), Nitrogen Dioxide (NO₂), and Sulfur Dioxide (SO₂). Using Sentinel-5P satellite data (2020–2024) processed in Google Earth Engine (GEE), spatial and temporal variations in pollutant levels were analyzed before and after a significant war event in April 2023. The methodology included data acquisition, preprocessing (e.g., cloud masking, spatial filtering), monthly averages computation, visualization, and statistical analysis using Google Earth Engine (GEE), ArcGIS Pro, and Microsoft Excel. Results showed a marked post-war increase in AOD levels, attributed to infrastructure destruction, fires, and diminished industrial oversight, alongside spatially consistent pollution patterns in some regions. CO concentrations exhibited an overall decline due to reduced industrial activities and transportation, though localized anomalies were linked to concentrated emissions. Similarly, NO₂ levels dropped significantly, reflecting reduced vehicular and industrial activities, while sporadic increases suggested localized emissions like generator use. SO₂ demonstrated mixed trends, with reduced mean levels but increased variability, indicating sporadic high-emission events linked to emergency fuel use or conflict-related disruptions. This study uniquely combines high-resolution satellite data with advanced spatial and temporal analysis techniques to reveal the nuanced and multi-pollutant impact of socio-political conflicts on air quality in Khartoum, providing novel insights into the environmental repercussions of armed conflicts. These findings highlight the profound impact of socio-political events on atmospheric pollution dynamics, underscoring the need for robust urban planning, targeted environmental monitoring, and policies to mitigate air quality deterioration and address public health concerns in conflict-prone regions. The study emphasizes the importance of satellite-based monitoring to provide critical insights into the environmental repercussions of socio-political upheavals.
Mitochondria are essential intracellular organelles that significantly influence various cellular processes, including metabolism, stress response, and cell fate. Their precise regulation is crucial for maintaining both organelle and cellular homeostasis. Wound healing is a complex, multifactorial process that relies on the coordinated actions of multiple cell types and numerous cellular mechanisms. Dysregulation in this process can lead to chronic wounds, which pose substantial challenges for healthcare systems and present limited treatment options due to their intricate pathogenesis. Recent research has increasingly focused on the role of mitochondria in wound healing, revealing their involvement in critical processes such as metabolism, apoptosis, and redox signaling. Mitochondrial dynamics play a vital role in wound healing by adapting to cellular demands and environmental cues. Moreover, mitophagy, the selective degradation of damaged mitochondria, is crucial for maintaining mitochondrial integrity and function during the healing process. Mitochondria are not only pivotal in energy production but also in calcium homeostasis and the generation of mitochondrial reactive oxygen species, which are essential for signaling during wound repair. As wound healing progresses through distinct yet overlapping stages mitochondria facilitate the energy demands of repair and contribute to cytoskeletal remodeling necessary for wound closure. Understanding the multifaceted roles of mitochondria in wound healing could lead to novel therapeutic approaches for chronic wounds. Future research should prioritize investigating mitochondrial dynamics and functions in human tissues to develop targeted strategies for enhancing wound healing outcomes.

The prevalence of gallbladder stones is higher in Chronic Kidney Disease (CKD) patients and it has been shown to increase with the advancement of the disease stage, from 7.7% in stage 1% to 21.3% in stage 5. Gallstone ileus is a rare complication which presents in just 0.3% - 0.5% of patients with cholelithiasis. A 61-year-old female patient, with a known case of CKD on maintenance hemodialysis, (thrice a week) with primary disease of hypertensive and diabetic nephropathy; presented with multiple episodes of loose stool, vomiting, and diffuse abdominal pain for 2 days. Abdomen Ultrasonography (USG) was suggestive of intestinal obstruction. CT abdomen with oral contrast revealed grossly dilated jejuna loops with air-fluid levels and transition zone in the pelvis, in distal jejunal loops/proximal ileum with ovoid intraluminal filling defect cystic polyp and collapsed bowel loops. The patient underwent exploratory laparotomy in view of persistent small bowel obstruction. Resection and anastomosis of the mass-bearing small bowel segment were performed. On cutting and opening the specimen, a large stone was revealed. This gallstone was causing bowel obstruction-gall stone ileus. A gallstone 2.6 cm x 2.1 cm has traversed through a cholecysto-duodenal fistula and got stuck in the proximal ileum, causing small bowel obstruction. The lesson learned is uraemia can cause gastrointestinal symptoms like anorexia, abdominal pain, vomiting, and ileus and hence mimic serious differentials of the acute abdomen like gallstone ileus. Thus no stone should be left unturned especially when the prevalance of gallstones is high in chronic kidney disease patients.
Methylenetetrahydrofolate Reductase (MTHFR) is an important enzyme of the folate cycle, which is required to convert 5,10-methyltetrahydrofolate into 5-methyltetrahydrofolate (5-methylTHHF). 5-methyl THF is a methyl group donor for several cellular methylation processes. It also donates methyl group for the conversion of homocysteine into methionine, the higher concentration of which is toxic. MTHFR gene C677T polymorphism is clinically important polymorphism and the variant MTHFR (A222V) enzyme has reduced activity, hence increasing the requirement for folic acid. Less conversion of folate to 5-methyl-THF due to C677T polymorphism results in a higher plasma concentration of homocysteine (hyperhomocysteinemia). Individuals having C677T polymorphism are susceptible to various diseases, including reproductive problems like male infertility, polycystic ovary syndrome, Recurrent Pregnancy Loss (RPL), Preeclampsia (PE), placental abruption, and adverse pregnancy outcomes. MTHFR C677T polymorphism mimics folate deficiency, and folate is required for DNA synthesis, repair, methylation, and proper chromosome segregation, and all these processes are important for foetal growth and normal development. Methylation and demethylation processes control the gene expression of about 45% of human genes. Impaired methylation influences the expression of genes involved in the regulation of hormones, spermatogenesis, and oogenesis. In males, oxidative stress damages sperm DNA decreases sperm motility, and may impair fertilization capability. In pregnant women, hyperhomocysteinemia increases oxidative stress and inflammation within the placenta, which causes damage to placental tissue, impairs its function, and disrupts foetal development. Further, hyperhomocysteinemia (HHcy) is embryotoxic and neurotoxic and is responsible for congenital anomalies in the foetus. This review supports the idea that MTHFR C677T polymorphism is associated with an increased risk for male infertility, PCOS, RPL, PE, and congenital anomalies. This review may provide a clue toward a better understanding of the correlation between the MTHFR C677T polymorphism and its detrimental effects on human reproductive health.

Background: Nutritional awareness among caregivers plays a crucial role in ensuring optimal pediatric health and development. This study assesses the level of nutritional awareness among caregivers and examines its association with demographic factors.Objectives: This study aimed to assess the knowledge and awareness of caregivers regarding dietary advice and complementary feeding for children under five years at Wad-Albasheir Family Health Center, Um-Bada locality.Methods: A cross-sectional study was conducted among caregivers in selected healthcare facilities. Data were collected using a structured questionnaire and analyzed using SPSS version 23. Descriptive statistics and chi-square tests were used to evaluate associations between variables.Results: The majority of caregivers demonstrated moderate awareness (60%), while 22.5% had low awareness and 17.5% had high awareness. A significant association was observed between education level and awareness (p < 0.05). Employment status also influenced awareness levels, with employed caregivers showing higher knowledge scores.Conclusion: Nutritional awareness is a key determinant of pediatric health. Strengthening educational programs targeting caregivers can improve child nutrition outcomes.
Cystoid Macular Oedema (CMO) is a condition characterized by fluid accumulation in the macular region of the retina, leading to the formation of cyst-like spaces. This edema often results in visual impairment and is associated with various ocular and systemic conditions, including surgery, inflammation, or medication use. The authors present a case where Cystoid Macular Oedema (CMO) occurred after commencing topical bimatoprost in a pseudophakic patient with primary open angle glaucoma. The macular oedema was treated effectively with a combination of non-steroidal and steroidal topical drops. This case report shows a possible correlation between bimatoprost and CMO, in a patient with no recent confounding risk factors known to contribute to CMO . The recommendation from this report is that all patients treated with topical bimatoprost drops should have a baseline macula OCT examination and a repeated OCT examination 8 weeks after initiation of treatment, to facilitate early detection of CMO.
