As the pandemic continues, it will be crucial to promptly evaluate the potential of any new variant to cause severe illness. The severity of the latest Omicron sublineages, including BA.5, XBB, BQ.1.18, BA.2, BA.2.75, and EG.5.1, is currently under assessment. This system provides valuable and essential information for rapidly assessing the threat posed by new versions of the virus.

Towards A 21st Century Systematize the Ideas; COVID-19, Sustainability and Discourse of SDG, (Sustainable Development Goals), The Cities and Housing Models

The research creates a theoretical basis for examining the metamorphic changes and transformations in urban and housing planning with the interaction created by the latest epidemic in the world, which is moving towards a period related to climate problems. The first quarter of the 21st century witnessed an unexpected event: Humanity found itself in the middle of a pandemic considered temporary, it would be permanent and change the world dramatically. The post-COVID-19 period has led to significant and permanent changes in our lives, from urban planning to housing typologies. The study aims to understand the changing dynamics in the post-pandemic period that converges on the same goal with SDG, (Sustainable Development Goals), as continuity by updated dialogues, and discourses on literature. This research focuses on the SDG, (Sustainable Development Goals), and the post-pandemic period, analysis of two phenomena separated, analogous paradigms or intertwined. The findings of the public perception are there is a distinction between sustainability and pandemics considered dissimilar processes, they contain similarities. Contrary to common ideas, the pandemic was a catalyst for sustainability, and these two concepts contain analogous ideas and principles. In conclusion, it is revealed that they both concentrated on similar ideas; SDG, (Sustainable Development Goals) is “health”, and pandemic is the “healthy city” concept like; clean air, zero carbon, living healthy and safely in airy and green areas, etc.

Statistical Mathematical Analysis of COVID-19 at World Level

Worldwide, statistical data of people infected by COVID-19 has been taken until March 29, 2023, which, when correlated, showed a predictive logistic  model. The purpose was to determine the predictive model, which was acceptable, in such a way that the proportionality constant and the correlation and determination coefficients are of great importance to estimating epidemiological and pandemic data; coinciding with what was reported by other authors. Bearing in mind that a mathematical model is a mathematical description through a function or equation of a phenomenon in the real world; whose purpose is to understand infections and make predictions for the future. The stages were: to model the number of people infected as a function of time, formulate, and choose the logistic model, determine the model and obtain mathematical conclusions, and make predictions (estimates) about the number of people infected by COVID-19 worldwide. The logistic model was derived to predict the speed of people infected by COVID-19 and the critical time (tc = 733 days) for which the speed was maximum (1694,7209 infected/day). The Pearson correlation coefficient for the time elapsed (t) and the number of people infected (N) worldwide, based on 32 cases, was r = -0.88; the relationship between time and those infected is real, there is a “very strong correlation” between the time elapsed (t) and the number of people infected (N) and 77.03% of the variance in N is explained by t.

COVID-19 Pneumonia in Pregnancy: A Retrospective Study on Maternal and Neonatal Outcomes

Objective: To identify risk factors among pregnant with COVID-19 for adverse outcomes related to disease severity, maternal mortality, and morbidity.Materials and methods: In this retrospective study, 45 pregnant patients with COVID-19 pneumonia were confirmed by RT-PCR. The inclusion criteria were pregnant patients diagnosed with COVID-19 confirmed by RT-PCR and hospitalized in the gynecology-obstetrics and intensive care unit. Exclusion criteria were non-pregnant patients and pneumonia cases with unconfirmed COVID-19 causes. The study used SPSS software to analyze the data. Results: Our study recorded 45 cases of SARS-CoV-2 infection in pregnant women over 2.5 years. The age group most affected was 20-35 years, with 75% of cases. 57% of patients had no known comorbidities. 88.8% of patients were symptomatic at diagnosis. Almost 30% of patients required admission to the ICU, with 60% requiring oxygen supplementation. The study recorded 36 live births (80%), of which 26 cases (72.2%) required no further care and had a favorable outcome.Conclusion: Pregnant women with medical conditions are at higher risk of severe COVID-19, which can cause respiratory distress syndrome and impact delivery and neonatal outcomes. Preventive measures are important.

Alcohol and Substance Abuse in the General Population during the COVID-19 Pandemic: Results of the COMET-G International Study

The global impact of the COVID-19 pandemic on mental health and substance use behaviors has sparked extensive research efforts. The COMET-G international study, organized by the Department of Medicine and the Rectorate of the Aristotle University of Thessaloniki in collaboration with the World Psychiatric Association, delved into these issues. Running from March 2020 to April 2021, the study collected responses from 55,589 individuals across 40 countries. Through a comprehensive questionnaire, participants provided insights into their mental state, attitudes toward the pandemic, and the resultant changes in their personal and daily lives. Findings revealed, among other things, significant patterns of change in substance use, with notable correlations between reduced usage and the severity of lockdown measures among non-binary individuals. Mental health history emerged as a strong predictor of substance use changes, with influences from anxiety disorders, depression, and self-harm. Additionally, family and social dynamics, including economic expectations and household composition, significantly shaped substance use behaviors during lockdowns. Given these findings, the development of comprehensive approaches targeting the adverse effects of the pandemic on individual behaviors and general welfare is crucial.

A Retrospective Analysis of Maternal Mortality in a Tertiary Care Centre at District Level

Background: Maternal mortality is a very sensitive indicator of our health system and services. Epidemiological data about maternal mortality is a vital requirement in every setting not only to design interventional programs but also to identify gaps in the existing structures, to reduce the ratio favorably. This study was an attempt to shed light on the mortality rate in our hospital, to analyze the epidemiological aspects, causes of maternal mortality, and types of delay, and to suggest recommendations for improvement. Methods: A retrospective study was done at a tertiary care center at the district level from January 2022 to December 2022. Demographic data and details of selected parameters were obtained from maternal death review forms and case records. Data analyzed and presented.Results: The majority of the maternal deaths could be attributed to direct obstetric causes like Eclampsia (29.41%), Pre-eclampsia (20.59%), and anemia (20.59%) followed by hemorrhage (14.71%), and septicemia (11.76%). Medical conditions contributing to death included liver disorders (5.8%), COVID-19 infection (5.8%), and cardiac disorders (2.9%). Conclusion: It is evident that the maternal mortality ratio is significantly high, with a majority of the deaths occurring due to avoidable causes. Leading contributory factors were due to delay in seeking help or delay in referral. High-risk cases must be identified at the earliest and referred to higher centers for management from the first trimester itself. Generating awareness among the common public and counseling the gravidas and their caretakers throughout pregnancy is equally important to prevent maternal mortality and near-miss cases.

Investigating Anti-Bacterial and Anti-COVID-19 Virus Properties and Mode of Action of Pure Mg(OH)2 and Copper-infused Mg(OH)2 Nanoparticles and Coated Polypropylene Surfaces

Robust anti-microbial surfaces that are non-toxic to users have widespread application in medical, industrial, and domestic arenas. Magnesium hydroxide has recently gained attention as an anti-microbial compound that is non-toxic, biocompatible, and environmentally friendly. Here we demonstrate melt compound and thermally embossed methods for coating polypropylene with Mg(OH)2 nanoplatelets and copper-infused Mg(OH)2 nanoplatelets. Polypropylene articles coated with Mg(OH)2 nanoplatelets and copper-infused Mg(OH)2 nanoplatelets exhibit a log 8 kill of E.coli within 24 hours. In addition, Mg(OH)2 NPs suspension, at 0.25% reduced SARSCoV-2 virus titers in the solution by 2.5 x 103 PFU/mL or 29.4%, while the Cu-infused Mg(OH)2 NPs suspension, at 0.25% reduced titers by 8.1 x 103 PFU/mL or 95.3%. Fluorescence microscopy revealed that reactive oxygen species (ROS) are produced in bacteria in response to Mg(OH)2 and Cu-infused Mg(OH)2 nanoplatelets which appears to be an important but not the sole mode of anti-microbial action of the nanoplatelets. Plastics with anti-microbial surfaces from where biocides are non-leachable are highly desirable. This work provides a general fabrication strategy for developing anti-microbial plastic surfaces.

Bridging the Gap: Challenges and Solutions in Online Nephrology Education

The outbreak of COVID-19 has created an unprecedented situation that has accelerated online education in areas like nephrology, which are traditionally served by onsite training. Efforts to educate clinicians remotely provide convenience and flexibility but may leave many open questions when it comes to how well-trained professionals maybe after completing their programs online. This review paper has emphasized the deleterious effects of online nephrology education and identified a number of ways in which it can be problematic such as lack of hands-on training, less patient interface, lack of proper supervision, and inconsistent quality of programs. The ability to make a difference, however, is further restricted by technological barriers and resource constraints. Whilst these programs tend to focus on creating knowledgeable graduates, they often fail to provide the clinical exposure necessary for preparedness. Hybrid programs - which combine online courses with clinical training in person are required to deliver high-quality nephrology care.

Post-COVID-19 Era, 15th Minutes City New Urban Model Changing Housing Design and Models

This research centered on the transition from sustainability to the post-COVID-19 era, significantly altered and transformed cities, city plans, and housing models holistically approach. Although the academic world concentrating on sustainable urban and housing design since the 1990s, the pandemic has emerged as a critical paradigm shift in context since 2019. During the COVID-19 period the Paris city 15 minutes concept, first introduced by Paris Mayor A. Hidalgo initially sparked controversy as temporary now considered permanent in Paris recently, Oxford. In the post-pandemic era, sustainability has become significant on the global agenda, shaping cities, city models, and residences through profound and radical changes. Some efforts are supported by Climate-change conferences in the world and subsequently some determinations like the Paris Agreement and UN declarations that guide more livable environments and houses. Due to COVID-19 the newly emerged conditions and compulsory changes that have taken place in the cities the social isolation, distance living, and remote working led to radical changes in the living style, environment, city plans, housing models, and typologies holistically. Although many publications have been published in the last few years about the pandemic and the Paris City 15 minutes concept, they only address the city and urban scale solutions. There is no research with a holistic approach that relates the subject from the urban scale to the architectural scale, housing, and blocks. Within the 15-minute concept, searched, developed, innovative, and sustainable urban and housing-oriented suggestions for cities in the future are presented at the end of the research. In conclusion, it is revealed that the issue of sustainability has gained importance with an increasing momentum in the world agenda, witnessed the radical changes in cities and residences, in the post-COVID-19 period in the last 2-3 years.

Seminal Extracellular Traps in SARS-CoV-2 Infection

Introduction: Extracellular Traps (ETs) are fibers composed of chromatin and cytoplasmic proteins, which can trap and kill pathogens by the phenomenon called ETosis. They are released by neutrophils, macrophages, and monocytes, and can be found in semen. The aim of this presentation is to evidence of the indirect effect of SARS-CoV-2 in semen by ETs.Patients and methods: Experimental design: retrospective descriptive observational study.Semen samples from two groups were studied following WHO guidelines: 1) SARS-CoV-2 infected donors (n: 5; at 7, 15, 30, 60, and 90 days after PCR diagnosis); 2) COVID-19 positive patients assisted in our laboratory between 2021 and 2022 (n: 70). They were observed in fresh and in Papanicolaou-stained smears by CASA and light microscopy; the presence of macrophages, spermiophages, ETs and hyperviscosity were recorded while neutrophilic concentration was calculated. Two control groups were designed: a) Patients belonging to group 2, studied before de pandemia (n: 13); b) Culture-negative semen samples (n: 28).Results: In the first group, ETs were observed in all the samples, while only 18% had leucospermia. Macrophages, spermiophages, and hyperviscosity were recorded in 68%, 27%, and 36% of the studied cases respectively.In the second group, ETs were present 100% in the acute phase (< 90 days after diagnosis) and decreased to 71% in the later stage (90 to 270 days). The trapped sperm were non-progressive motile or immotile alive or dead.No traps were found in either control group.Conclusion: In our study ETs were the most sensitive seminal marker of SARS-CoV-2 infection.

COVID-19, Long COVID, and Gastrointestinal Neoplasms: Exploring the Impact of Gut Microbiota and Oncogenic Interactions

Background: The impact of COVID-19 and long-term COVID-19 on gastrointestinal neoplasms remains underexplored. The current review investigates the potential link between these conditions and the role of gut microbiota in mediating oncogenic processes. Dysbiosis, characterized by alterations in gut microbial composition, may exacerbate inflammation and immune dysregulation, contributing to cancer development.Methods: A comprehensive literature review was conducted using databases including PubMed, Scopus, Embase, SciELO, and Web of Science. Inclusion criteria encompassed studies published between 2020 and 2024 that explored the intersection of COVID-19, long-term COVID-19, and gastrointestinal cancers. The articles were critically appraised for quality and relevance, and data were synthesized to elucidate common mechanisms and outcomes.Results: The review identifies several mechanisms by which gut microbiota may influence cancer risk in COVID-19 patients. Persistent inflammation, oxidative stress, and immune dysfunction observed in Long COVID were associated with dysbiosis. Specific microbial metabolites, such as secondary bile and short-chain fatty acids, were implicated in promoting tumorigenesis. Comparative analysis of studies suggests that SARS-CoV-2-induced dysbiosis may heighten susceptibility to gastrointestinal cancers, particularly in patients with prolonged post-infection symptoms.Conclusion: The findings underscore the need for further research to clarify the role of gut microbiota in cancer development among COVID-19 patients. These mechanisms could inform preventative strategies and therapeutic interventions, particularly for those experiencing COVID. The review highlights gaps in current knowledge and advocates for longitudinal studies to assess the long-term effects of COVID-19 on gastrointestinal health.

Coronaviruses have reached at Pre-elimination Stage with Nine Amino Acid Spike Deletions and Forty-nine Nucleotide 3’-UTR Deletions

Background: Human 30kb coronaviruses entered through the ACE-2 receptors causing fibrosis of the lungs and causing six million deaths worldwide. Here, we have investigated the mutations, deletions and insertions of the recent JN.1 omicron coronaviruses to demonstrate that coronaviruses have reached the pre-elimination stage. Methods: We multi-aligned the genomes of recent JN.1 variants using NCBI Virus Portal and CLUSTAL-Omega. The spike proteins are multi-aligned using MultAlin software and CLUSTAL-Omega.Results: The 17MPLF spike insertion was confirmed to compensate 24LPP, 31S, 69HV, 145Y, 211N and 483V deletions. The 49nt deletions in the 3’-UTR were found in 4997 JN.1 sequences although 26nt deletion was initiated previously in JN.1 as well as BA.5, BF.7, BQ.1 and XBB.1.5 omicron viruses. We first compare 3-D structures of spike proteins with or without 17MPLF four amino acids insertion and nine amino acids deletions using SWISS MODELLING. The JN.1 viruses caused a more stable trimeric spike involving Thr342, Lys436, Lys440, His441, Ser442, Gly443, Tyr445, Lys479, Ser489, Tyr490, Arg493, Pro494, Thr495, and Gln501 amino acids to interact with ACE-2 receptors. The FLiRT spike mutations were found in most KP.2 variants and other changes occurred at the NH2 terminus.Conclusion: We claimed that pre-death changes were initiated in JN.1 COVID-19 lineages and computer simulation showed that the Howard spike with 17MPLF spike insertion appeared more stable than the Oppentrons-spike without 17MPLF insertion. Surely, conflicts of COVID-19 spike sequences must be resolved.

Association Between Electrocardiographic Abnormalities and In-Hospital Adverse Outcome in COVID-19 Patients

Background: SARS-CoV-2 is increasingly recognized for its cardiovascular complications. To address the knowledge gap in our region, this study investigated the relationship between electrocardiographic (ECG) features and in-hospital mortality among COVID-19 patients.Methods: A prospective cohort study was conducted involving 140 RT-PCR-confirmed COVID-19 patients at Dhaka Medical College Hospital from August 2021 to July 2022. Patients were divided into two groups based on ECG findings: normal (Group A) and abnormal (Group B). Clinical data and ECG parameters were analyzed using SPSS 24.0.Results: Patients with abnormal ECGs were older, more likely male, and presented with higher rates of dyspnea and palpitations. They were also at increased risk of severe COVID-19 and had longer hospital stays. In-hospital mortality was significantly higher in Group B (21.4% vs.4.3%). ST-T changes and atrial fibrillation were associated with increased mortality. Independent predictors of in-hospital mortality included ECG abnormalities and admission SpO₂ < 90%.Conclusion: ECG abnormalities are significantly associated with adverse outcomes in COVID-19 patients. Further large-scale studies are warranted to strengthen these findings.

Is Acupuncture Efficient for Treating Long COVID? Case Reports

Long COVID can be defined as a set of symptoms appearing more than 28 days after a documented acute COVID-19. Among them, extreme fatigue and troubles of moods are the most common. To verify that acupuncture can efficiently alleviate long COVID, a chronic disease partly escaping to other treatments, especially fatigue and troubles of mood. To collect data pertaining to the patients included in the study as described in the STROBE guideline. To diagnose the clinical conditions of the patients using the Nan jing in order to choose the meridian and the points to be toned or dispersed depending on the observation of tongue and pulse. Acupuncture can greatly improve the troubles of patients suffering from long COVID after a reduced number of sessions (1 to 4).

Understanding Burnout Rates for Clinicians and Physical Symptoms of Allostatic Load

Burnout among clinicians is a pressing concern worldwide, manifesting as emotional exhaustion, depersonalization, and reduced professional efficacy. This article explores the intersection of burnout and allostatic load, the physiological burden resulting from chronic stress, to elucidate the consequences for healthcare providers. Burnout impacts clinicians’ mental and physical health, leading to compromised patient care, reduced job satisfaction, and increased attrition rates. A comparative analysis of recent scholarly works reveals converging evidence on systemic contributors such as excessive workloads, inadequate support systems, and organizational inefficiencies, further compounded by individual vulnerabilities like lack of resilience. These challenges have been exacerbated by the COVID-19 pandemic, which has significantly increased stress levels among healthcare professionals globally. This article synthesizes insights from studies conducted between 2020 and 2024, emphasizing the need for holistic approaches to mitigate burnout. It highlights the physiological underpinnings of allostatic load, including chronic dysregulation of stress-response systems that predispose clinicians to adverse health outcomes like cardiovascular diseases, immune dysfunction, and mental health disorders. To address these issues, the article proposes a multidimensional strategy encompassing organizational reforms, evidence-based interventions, and policy advocacy. Recommendations include reducing administrative burdens, fostering supportive work environments, and integrating wellness programs targeting both systemic and individual stressors. Limitations and directions for future research emphasize the importance of inclusive, longitudinal studies focusing on diverse populations to develop tailored solutions. This comprehensive approach aims to enhance clinician well-being and improve healthcare outcomes globally.

Dengue Epidemic during COVID-19 Pandemic: Clinical and Molecular Characterization – A Study from Western Rajasthan

The concurrent emergence of dengue fever and the COVID-19 pandemic posed significant challenges to India’s healthcare system, particularly in Western Rajasthan, a region characterized by its arid climate and unique socio-demographic conditions. This study aimed to investigate the clinical and molecular characteristics of dengue during the COVID-19 pandemic, focusing on trends, diagnostic challenges, and serotype distribution. Conducted at Dr. S.N. Medical College, Jodhpur, in 2021, the study included 550 dengue-positive patients confirmed via rapid diagnostic tests and further analyzed using Dengue NS1 antigen and IgM antibody ELISA. Molecular characterization was performed using RT-PCR for serotyping.The results revealed a male predominance (72.36%) and a higher incidence in the 21–30-year age group (39.09%). Urban areas accounted for 67.73% of cases, with significant NS1 and IgM positivity (p = 0.042 and p = 0.004, respectively). Most cases (86.91%) were managed outpatient, though IgM positivity was significantly higher among hospitalized patients (19.19%, p < 0.001), indicating severe or prolonged infections. Platelet counts were above 100,000/mm³ in 86.91% of cases, with only 0.37% showing critically low counts (< 20,000/mm³). Seasonal analysis showed a peak in October (n = 325), correlating with post-monsoon vector breeding. Serotyping identified DENV2 as the dominant strain (97.42%), associated with severe dengue manifestations, including Dengue Haemorrhagic Fever (DHF).The study highlights the dual burden of dengue and COVID-19, emphasizing the need for enhanced vector control, improved diagnostic strategies and public health interventions during overlapping outbreaks. The predominance of DENV2 underscores the importance of serotype-specific surveillance and preparedness to mitigate future dengue epidemics in the region.
