Prescribing Inertia or Not? Quantitative Investigation of Loop Diuretics Prescribing after Palliative Care Consultation among Patients with Heart Failure

Purpose: Loop Diuretics (LD) are the first-line pharmacotherapy to address Heart Failure (HF)-associated edema and dyspnea. However, LD causes frequent urinary, resulting in inconvenience and possibly undermining the quality of life. While prescription adjustment is an essential part of Palliative Care Consultation (PCC), it remains unclear how PCC affects the deprescribing of diuretics for adults with HF. Methods: We conducted a pre-post analysis of the percentage of HF patients who were prescribed LD in a national Electronic Health Record (EHR) database 12 months before and after the first PCC. The difference in prescription rates between the periods was determined. Adjusted associations of post-PCC LD prescription with pre-PCC LD prescription and patient’s characteristics, insurance, provider type, and clinical factors were quantified.Results: From 2010 to 2018, 5,969 patients with newly diagnosed HF received at least one PCC, among whom 2,539 (42.5%) were prescribed LD before and 1,552 (26.0%) after their first PCC. Despite a decrease in LD prescription rate encompassing the date of PCC, post-PCC LD prescribing was strongly associated with pre-PCC prescribing (aOR[95%CI] 3.2[2.8,3.7]) and varied by age at first PCC, year of HF diagnosis (aOR[95%CI] 2.1[1.9,2.4]) and months from HF diagnosis to first PCC. While our finding demonstrates reduced polypharmacy associated with PCC, the strong association between pre- and post-PCC indicates reverse therapeutic inertia. Future research should investigate the benefits and costs of polypharmacy among specific patient groups to help develop personalized treatment for HF.

Dentistry is a challenging and demanding physical profession, and this impacts not only the quality of the task outcomes but also the professional’s quality of life. The nature of dentistry demands prolonged static and awkward seated postures, frequent motions, positioning struggles, and working distances to the oral cavity for strategic procedure management, which become cumulative challenges over the working days, prone to discomfort, pain, or injuries, and musculoskeletal disorders for the operator.The limitations of the naked eye in dentistry, particularly in terms of visual information and resolution, underscore the need for visual aids. These aids are crucial for achieving the precision required for accurate procedures. Also, visual aids, such as magnifying loupes and the operative microscope, have been in the market for a while, and their proper use has shown ergonomic benefits that promote a neutral working posture and changes in work practice-oriented to control unbalanced awkward posture and motions.In this study, the Rapid Upper Limb Assessment RULA compares the posture between naked-eye dentistry, conventional loupes, and operative microscope, and with the observational assessment, determine the risk of developing musculoskeletal disorders and the need for priority of change. The Rodgers muscle fatigue analysis RMFA is used to predict fatigue in the muscular regions of the body during specific dental tasks.

COVID-19, Long COVID, and Gastrointestinal Neoplasms: Exploring the Impact of Gut Microbiota and Oncogenic Interactions

Background: The impact of COVID-19 and long-term COVID-19 on gastrointestinal neoplasms remains underexplored. The current review investigates the potential link between these conditions and the role of gut microbiota in mediating oncogenic processes. Dysbiosis, characterized by alterations in gut microbial composition, may exacerbate inflammation and immune dysregulation, contributing to cancer development.Methods: A comprehensive literature review was conducted using databases including PubMed, Scopus, Embase, SciELO, and Web of Science. Inclusion criteria encompassed studies published between 2020 and 2024 that explored the intersection of COVID-19, long-term COVID-19, and gastrointestinal cancers. The articles were critically appraised for quality and relevance, and data were synthesized to elucidate common mechanisms and outcomes.Results: The review identifies several mechanisms by which gut microbiota may influence cancer risk in COVID-19 patients. Persistent inflammation, oxidative stress, and immune dysfunction observed in Long COVID were associated with dysbiosis. Specific microbial metabolites, such as secondary bile and short-chain fatty acids, were implicated in promoting tumorigenesis. Comparative analysis of studies suggests that SARS-CoV-2-induced dysbiosis may heighten susceptibility to gastrointestinal cancers, particularly in patients with prolonged post-infection symptoms.Conclusion: The findings underscore the need for further research to clarify the role of gut microbiota in cancer development among COVID-19 patients. These mechanisms could inform preventative strategies and therapeutic interventions, particularly for those experiencing COVID. The review highlights gaps in current knowledge and advocates for longitudinal studies to assess the long-term effects of COVID-19 on gastrointestinal health.

Comparative Analysis of Water Wells and Tap Water: Case Study from Lebanon, Baalbeck Region

Water deficit is a fundamental factor in public health and economic growth. Water supply and population growth are directly linked to water demand. The physio-chemistry and microbiology analysis of water is utmost significance in dietary requirements. Drinking water has the main concern especially it affects food security. This study includes a number of representative sites where 24 water samples (from wells, reservoirs and tap water) were analysed. These sites are located in the western villages of Baalbeck, the main city of the Bekaa Plain in Lebanon where the analysed water is used mainly for domestic needs and for irrigation. This study  investigates the physiochemical and microbiological properties. Among the selected sites, Hawsh Barada site shows a strong contamination by nitrate. At the Nabi Rashaded (tap), Beit Shema and Bednayel (borehole and tap), contamination above the norm by zinc ion was noted. From a microbiological point of view, Hawsh barada, Nabi-Rashadeh, Hawsh-bay, and Beit shema are markedly polluted and do not meet the standard for drinking water. Hence, water quality in Hawsh barada, Nabi-Rashadeh, Hawsh-bay, Beit shema and Bednayel are not suitable for drinking, and this must be informed to decision makers who can act implementing environmental controls for health protection in the studied region.

Neurovascular Shifts, Sensory Sensitivity, and PMDD in Autistic Women: Exploring Blood Flow Redirection, Mood Dysregulation, and Pain Tolerance during Menstruation

This article examines the relationship between Premenstrual Dysphoric Disorder (PMDD), neurovascular dynamics, and sensory sensitivities in autistic women during menstruation. The redirection of blood flow to the uterus during the menstrual cycle has been found to exacerbate cerebral perfusion deficits in neurodivergent individuals, particularly in the Prefrontal Cortex (PFC), which contributes to the mood dysregulation and emotional instability characteristic of PMDD. Autistic women, who often exhibit heightened sensory sensitivities, experience intensified discomfort during menstruation, as sensory overload and altered pain perception compound the emotional challenges of PMDD. These findings emphasize the need for neurodivergent-friendly menstrual products that mitigate both physical and emotional discomfort. Additionally, innovations using biodegradable materials, smart fabrics, and custom-fit menstrual solutions are discussed as potential breakthroughs to improve the quality of life for autistic women managing PMDD. This research highlights the importance of addressing both neurobiological and sensory aspects when designing interventions for PMDD in neurodivergent populations.

Forecasting Soil Moisture in Caragana Shrubland Using Wavelet Analysis and NARX Neural Network

It is important for sustainable use of soil water resource and high-quality development to forecast the soil moisture in forestland of water-limited regions. There are some soil water models. However, there is not the best model to forecast the change of soil moisture in the caragana shrubland. In this paper, the plant water relationship has been investigated at the same time for a long term in the caragana shrubland of semiarid region of the Loess Plateau of China. The data of soil moisture was divided and then NARX neural network was used to build model I and model II. For model I, low frequency component was the input variable, and for model II, low frequency component and high frequency component were predicted. The results showed the average relative error for model I is 3.5% and for model II is 0.3%. The average relative error of predicted soil moisture in 100 cm layer using model II is 0.8%, then soil water content in the 40 cm and 200 cm soil depth is selected and the forecast errors are 4.9% and 0.4%. The results showed that using model II to predict soil water is well Predicting soil water using model II will be important for sustainable use of soil water resource and high-quality development.

However, because of better plant species and varieties, overuse of fertilizer and overdose of pesticide alters the plant resources relationship, resulting in soil degradation, vegetation decline, and crop failure or waste of resources, which influence the quality of fruit and benefit and are unfavorable for the sustainable utilization of nature resources and crops high-quality development. After a couple of years of study, the results showed that Agricultural development went into agricultural high-quality development in 2017, which is to use effective measures or methods to maximize yield and benefits and meet the need of people's need for better Agricultural produce and a better life. The theory foundation of the sustainable utilization of natural resources and Agriculture high-quality production is the resources use limit by plants, vegetation carrying capacity, and critical period of plant resources relationship regulation. The methods of Agriculture high-quality development are to select excellent plant species or varieties based on site conditions and market needs before planting crop young plants and sowing seeds, adopt appropriate initial planting density when planting crop young plants and sowing seeds, and regulate the relationship between plant growth and resources need in the process of crop production in a timely and appropriate amount, especially the chemical fertilizer and pesticide to obtain the maximum yield and benefits and realize the sustainable utilization of natural resources and achieve agricultural high-quality development.

Physiotherapy Undergraduate Students’ Perception About Clinical Education; A Qualitative Study

Background: Clinical education is an important component of physiotherapy education programs. It provides insights to the students in the fields of physiotherapy practice, develops leadership, and enhances their clinical skills in patient assessment, examination, diagnosis, treatment, planning, and intervention selection. The dimension of physiotherapy education is in the developing phase in Nepal. For further growth, the experience and the perception of the students are very important. Through this, the facilitators and barriers in physiotherapy clinical education need to be recognized. Therefore, this study aimed to explore the facilitators and barriers in physiotherapy clinical education in Nepal from a student’s perspective.Objectives: The main objective of this study was to identify the perception of physiotherapy undergraduate students about their clinical education and to find out the facilitators and barriers in clinical education.Methods: A qualitative research using focused group discussion was conducted. Ethical considerations were taken from respective departments and IRC approval was taken. BPT 3rd year, 4th year, and intern students of KUSMS were called to participate in the study. After the consent from the participants, focus groups of 6 participants (2 participants from each 3rd year, 4th year, and intern) were formed. In five FGDs, data saturation was observed and no further FGDs were conducted. FGDs were collected with audio tape recordings. The data were transcribed verbatim and translated into the English language. Then data coding was done and themes and sub-themes were generated from the codes.Results: Through the transcribed and translated data, seven themes were generated which were clinical education, student expectation, student attitude and behavior, roles and responsibilities of clinical educators, exposure, resources, and miscellaneous. These headings explained the perception of facilitators and barriers.Conclusion: The facilitators and barriers to physiotherapy clinical education were identified through student’s perspectives. These findings should be considered while preparing the guiding document for clinical education to enhance the quality of education.

Efficiency of Artificial Intelligence for Interpretation of Chest Radiograms in the Republic of Tajikistan

The article presents data from recent publications and own data on screening studies with interpretation of chest radiographs using artificial intelligence CAD (Computer-Assisted Diagnosis), which, according to WHO recommendations, provides more accurate clinical thresholds for deciding who needs to take a sputum test. Another aspect of the WHO recommendations is the cost-effectiveness of CAD as a tool for triaging patients with tuberculosis symptoms in low-income countries with a high incidence of tuberculosis. Compared with smear microscopy and GeneXpert, without preliminary sorting, the use of mobile digital X-ray machines equipped with a CAD tool reduces costs, allowing sorting of individuals suspected of having tuberculosis for testing on GeneXpert, while reducing the time to start tuberculosis treatment.Thus, conducting a study using portable X-ray machines using a CAD program is a low-cost and easy-to-implement method, does not require large funds, does not require separate rooms, is highly effective, has good image quality, allows you to quickly clarify individuals suspected of having tuberculosis, differentiating it from other pathological changes in the lungs.Our experience shows that machine analysis of chest computed tomography data, due to the higher resolution capabilities of the method and the absence of fundamental disadvantages of radiography, including the effect of shadow summation, the presence of “blind” zones, etc., is finding increasing application in both diagnostics and screening of respiratory diseases. Our use of this tool allowed us to identify additional new cases of phthisio-onco-pulmonary diseases in field conditions.

Plant growth, Yield and Leaf Nutritional value of Jute (Corchorus olitorius L.) as Influenced by Banana Peel levels under Salt Stress conditions in Coastal region of Cameroon

In the world, millions of hectares of cultivated land are affected by salt, making salinity a major constraint for plant production. The effects of different levels of banana peel on growth, yield, and chemical changes of jute (Corchorus olitorius L.) under salt stress were examined herein. Response of jute to applications of different levels of banana peel (5, 10, and 15 t ha-1), as well as water irrigation salinity at 50, 100, and 200 mM NaCl were evaluated under greenhouse conditions. The outcome uncovered that salinity caused significant reduction of plant growth and yield parameters, chlorophyll (at 21.6%), LRWC (at 18.9%), P (at 57.1%) and K (at 45.4%) content, while MDA content (at 351.8%), Na (at 266%), soluble proteins (70.2%), total phenolic (at 23.4%) accumulation showed an increase from 0 to 200 mM NaCl without BP application. The banana peel treatments (at 15 t ha-1 under 200 mM NaCl) diminished significantly damaging effects caused by salinity via a reduction in the Na (at 28.4%), total soluble sugars (at 17.8%), total flavonoids (at 20.1%), which enhanced number of leaves per plant (37.1%), plant height (at 19.8%), leave yield (at 41.4%), LRWC (at 12.8%), Mg (at 24.2%) and reduced the MDA content (at 20%), presenting a favorable effect in reducing the oxidative stress that emerged from salt stress. It could be concluded, that the application of 15 t ha-1 of BP was superior in promoting plant growth, yield, and nutritional quality than others under control and in the saline soils in this study. BP at 15 t ha-1 had a more reduced damage of salt stress effect on growth, yield, nutritional value, and use efficiency.

Sleep disorders represent a significant public health concern due to their widespread prevalence, impact on overall health, and the economic burden they impose. These disorders encompass a broad spectrum of conditions, ranging from insomnia and obstructive sleep apnea (OSA) to narcolepsy, restless legs syndrome (RLS), and parasomnias. They are often associated with comorbidities such as cardiovascular diseases, metabolic dysfunctions, and mental health disorders, making their identification and management critical.The publication of this work is of high interest as it contributes to the expanding body of literature focused on understanding the complex interplay between sleep disorders and health outcomes. By presenting detailed case reports, this study provides valuable insights into the diagnostic challenges, treatment modalities, and potential avenues for personalized interventions in sleep medicine. Case reports are particularly important in this field as they shed light on unique presentations and rare conditions that might otherwise go unnoticed in large-scale epidemiological studies. From an epidemiological perspective, sleep disorders are highly prevalent globally. According to the World Health Organization (WHO), approximately 30% - 45% of the global population experiences sleep disturbances. Obstructive sleep apnea, for instance, affects nearly 1 billion individuals worldwide, with varying prevalence across age, gender, and geographic regions. Insomnia affects roughly 10% - 30% of adults, with rates as high as 50% - 60% in older populations.Meanwhile, narcolepsy, though rare, is estimated to affect 1 in 2,000 people in the general population. These statistics underscore the pressing need for enhanced diagnostic methods, improved treatment strategies, and comprehensive patient management. By detailing real-world cases, this publication aims to bridge the gap between clinical observations and broader scientific understanding. The insights gained from these case studies have the potential to inform future research directions, improve clinical practices, and ultimately enhance patient outcomes in sleep medicine.Sleep disorders affect millions of individuals globally, disrupting physical, mental, and emotional well-being. Figures such as polysomnography (PSG) outputs and statistical trends provide visual insights into diagnostic and therapeutic interventions. Sleep disorders encompass a wide range of conditions that significantly disrupt sleep quality and overall well-being. Common disorders such as insomnia, obstructive sleep apnea (OSA), narcolepsy, and restless legs syndrome (RLS) affect millions globally, posing risks to physical health, mental stability, and cognitive performance. This study explores the clinical presentation, diagnostic approaches, and management of sleep disorders through the lens of detailed case reports and sleep study data.Polysomnography (PSG), the gold standard for sleep disorder diagnosis, plays a pivotal role in identifying abnormal sleep patterns, respiratory irregularities, and neural disruptions. Multiple sleep latency tests (MSLT) and actigraphy complement PSG, offering insights into disorders like narcolepsy and circadian rhythm abnormalities. This paper presents three representative case reports: chronic insomnia, severe OSA, and narcolepsy with cataplexy. Each case is analyzed in-depth, highlighting patient history, PSG findings, treatment interventions, and outcomes. For chronic insomnia, cognitive-behavioral therapy for insomnia (CBT-I) and pharmacological intervention resulted in marked improvements in sleep latency and efficiency. In the OSA case, continuous positive airway pressure (CPAP) therapy significantly reduced the apnea-hypopnea index (AHI) and alleviated daytime symptoms. The narcolepsy case demonstrates the efficacy of modafinil and sodium oxybate in managing excessive daytime sleepiness and cataplexy.Despite advancements, challenges persist in the field, including patient adherence to therapy, accessibility to specialized sleep studies, and the ethical implications of AI-driven diagnostic tools. Future research should focus on scalable, patient-centric approaches and the role of emerging technologies in enhancing diagnostic accuracy and treatment efficacy. This paper aims to contribute to the evolving understanding of sleep disorders, bridging clinical case insights with the broader implications for sleep health and research.

Development of Latent Fingerprints Using Food Coloring Agents

Aim and objective: The current study aims to explore the effectiveness of food dyes as potential agents for the formation of latent fingerprints on non-porous substrates. Introduction: The development of fingerprints is a long-established forensic technique crucial for identifying perpetrators. Traditional methods often use various powders to reveal latent fingerprints on different surfaces, but these methods can be costly and pose health risks when exposed to humans. Consequently, there is an increasing demand for alternative techniques that are cost-effective while still providing high-resolution fingerprint visibility. Materials and methods: In this study, Colormist Super whip food powder red, blue, orange, turquoise, and pink was used for the Development of Latent fingerprints on different non-porous surfaces (CD, Mobile screen, Glass bottle, Mirror, Steel bottle). Results: The ColorMist Super Whip edible powders proved successful in developing high-quality finger marks with visible level 1 and level 2 fingerprint details across all substrates, showcasing its effectiveness in latent fingerprint enhancement. Conclusion: Food dyes provide a simple, cost-effective, and eco-friendly method for developing latent fingerprints on nonporous surfaces. Red and black dyes consistently deliver clear ridge detail, while turquoise performs well on glass. Surface type plays a crucial role in dye effectiveness; making food dyes a practical, non-invasive alternative for on-site forensic use.

The institution’s preventive and supervisory activities contribute to reducing risks to the population’s health, preventing disease outbreaks, food poisoning, and the proliferation of pests, among other problems. Finally, the importance of exercising police power with transparency, impartiality, and respect for individual rights is emphasized, always aiming at the collective interest and the promotion of the common good. In summary, Health Surveillance plays an essential role in protecting public health, seeking to ensure that the population has access to safe products and services, contributing to the improvement of quality of life and disease prevention.

Satellite-Based Analysis of Air Pollution Trends in Khartoum before and After the Conflict

This study investigates the impact of socio-political disruptions on air quality in Khartoum, Sudan, focusing on key pollutants: Aerosol Optical Depth (AOD), Carbon Monoxide (CO), Nitrogen Dioxide (NO₂), and Sulfur Dioxide (SO₂). Using Sentinel-5P satellite data (2020–2024) processed in Google Earth Engine (GEE), spatial and temporal variations in pollutant levels were analyzed before and after a significant war event in April 2023. The methodology included data acquisition, preprocessing (e.g., cloud masking, spatial filtering), monthly averages computation, visualization, and statistical analysis using Google Earth Engine (GEE), ArcGIS Pro, and Microsoft Excel. Results showed a marked post-war increase in AOD levels, attributed to infrastructure destruction, fires, and diminished industrial oversight, alongside spatially consistent pollution patterns in some regions. CO concentrations exhibited an overall decline due to reduced industrial activities and transportation, though localized anomalies were linked to concentrated emissions. Similarly, NO₂ levels dropped significantly, reflecting reduced vehicular and industrial activities, while sporadic increases suggested localized emissions like generator use. SO₂ demonstrated mixed trends, with reduced mean levels but increased variability, indicating sporadic high-emission events linked to emergency fuel use or conflict-related disruptions. This study uniquely combines high-resolution satellite data with advanced spatial and temporal analysis techniques to reveal the nuanced and multi-pollutant impact of socio-political conflicts on air quality in Khartoum, providing novel insights into the environmental repercussions of armed conflicts. These findings highlight the profound impact of socio-political events on atmospheric pollution dynamics, underscoring the need for robust urban planning, targeted environmental monitoring, and policies to mitigate air quality deterioration and address public health concerns in conflict-prone regions. The study emphasizes the importance of satellite-based monitoring to provide critical insights into the environmental repercussions of socio-political upheavals.

Analysis of Acoustic Parameters of Voice Jitter and Shimmer in Children with Dysphonia after Laryngeal Surgery

The article conducted a study of computer acoustic analysis of Jitter and Shimmer voice parameters in children with dysphonia after laryngeal surgery. The study found that children with dysphonia had significantly lower preoperative Jitter and Shimmer scores (574359, 587, and 2.3) compared to healthy children (1316743.376 and 4.84). At 1 month after surgery, affected children showed a decrease in Jitter and Shimmer indices compared with these acoustic indices before surgery. However, at 2 months, there was a significant increase in Jitter and Shimmer scores, and at 3 months, Jitter and Shimmer scores were comparable to Jitter and Shimmer scores in healthy children without dysphonia. The results of the study show that the method of analyzing the acoustic parameters of the voice Jitter and Shimmer allows a phoniatrist to objectively assess the occurrence of a voice disease in patients with dysphonia, and this method is the most accurate criterion for determining the pathologies of dysphonia and determining the treatment program for the disease.

The Limitation of Determination Structure of the Agreed Rate of Return for Public-Private Partnership (PPP) Road Projects and Improvement in South Korea

Public-Private Partnership (PPP) road projects are being promoted worldwide to encourage road investments; however, research on the appropriate rate of return for these projects is insufficient. This is likely because the return on investment for PPP road projects is determined through negotiations between the government and the private sector, a process that remains confidential. The rate of return for PPP road projects is not only a key indicator for evaluating project performance but also a potential driver for future government financial subsidies. Therefore, research on the appropriate rate of return for PPP road projects is necessary for the sustainability of these projects.This study, based on the concept that both the government and the private sector share the anticipated risks of the project, quantifies six types of risks and proposes a model for predicting the appropriate rate of return. The model is able to explain approximately 68% of the cases in South Korea’s PPP road projects. The risk with the greatest influence on the rate of return was found to be an economic risk, while the least influential was regulation change risk.Due to the confidentiality of data related to PPP road projects, this study only utilized 54 cases from South Korea. It is hoped that a wider range of data will be collected and further research on the appropriate rate of return will continue to enhance the sustainability of PPP road projects.

Reduced Pain Scores during Indocyanine Green Lymphography by using a Different Preparation Formula

Purpose: The purpose of this study was to investigate whether a different indocyanine green lymphography formula can reduce the severe pain encountered during this investigation.Methods: Indocyanine green ICG lymphography frequently induces severe and intolerable pain in patients. It is very important to develop a relatively painless lymphography method. This helps reduce patients’ apprehension during examination. Our center conducted randomized control groups of 60 patients with limb lymphedema. Cases were divided into experimental and control groups. Patients in the experimental group were injected with indocyanine green solution containing 5% glucose, while patients in the control group were injected with indocyanine green solution with sterile water.VAS scores were recorded pre-injection, during, and at 10, 20, and 30 minutes post-injection. We recorded possible side effects, e.g. skin redness, itching, subcutaneous bleeding, bruising, severe allergic reactions, and other complications within 24 and 48 hours after imaging, and analyzed the pain score data. Results: The average pain score of the experimental group was 0-3 points, while the control group was 6-8 points. Disease staging was accurately assessed based on imaging results . No other serious complications occurred.Conclusion: Pain can be significantly reduced by the new modality. This method does not affect lymphography results and imaging quality, and there are no obvious serious complications.
